Search Instructions (United States)

To begin searching for a physician or provider located in the United States you may use any of the following options. To search for for a physician or provider located in another country please click here.

• Select a State (state search results can be filtered by city)
• Enter a Zip Code
• Enter a Zip Code & select a Travel Distance
• Enter a Doctor's Last Name (or partial name)

You may also use any combination of the above options. For example, when selecting the State "CA" and entering "A" for the Doctor's Last name, clicking the "Search Now" button will return only the physicians and providers that are located in California and have a last name starting with the letter "A."
